Licensed Clinical Social Worker (LCSW-A)

The Licensed Clinical Social Worker provides counseling to LNCHC patients as well as works within an integrated model of care with the clinical staff to improve the overall wellness of patients. The LCSW or LCSWA will meet weekly with the clinical staff to discuss the overall health of individual patients to improve health outcomes. Responsibilities specific to Case Management include assessment, identification of specific needs, and social service intervention.
